This interactive workshop will provide practical information and help you to make a start with your advance care planning. It’s important to start advance care planning early in life when you can speak and decide for yourself. As things change in your personal situation, health or lifestyle, you can go back and review your choices and, if needed, make changes to your documentation.

• Exploring what matters to you and what your priorities are for the last stage of life.
• How to have a conversation about your wishes with your family, loved ones and health care providers.
• What is involved in palliative care and when it should be offered.
• How to complete documents such as Advance Health Directive and Enduring Power of Guardianship.
