Self referrals to the Parkinson’s Nurse Specialist can ONLY be made for individuals that have a medical diagnosis of Parkinson’s or related condition (for example Multiple System Atrophy (MSA), Progressive Supranuclear Palsy (PSP) or Cortico-Basal degeneration (CBD).

Please note:

• The Parkinson’s Nurse Specialist service is not an emergency or crisis service. The Nurse Specialist service operates Monday to Friday during business hours.
• Completing this form does not automatically enrol you in the Parkinson's Nurse Specialist Service. Your referral will be reviewed for suitability, and if deemed appropriate, it will be forwarded to our Parkinson's Nurse. If your referral does not meet the criteria, we will contact you to discuss alternative options.
